Good Faith Estimate Notice
Under Section 2799B-6 of the Public Health Service Act, healthcare providers and health care facilities are required to provide a "Good Faith Estimate" of expected charges for items and services to patients who are not using insurance or who are self-paying for their care. This estimate helps ensure transparency in health care costs and provides you with the information needed to make informed financial decisions.
As a private-pay practice, we provide a Good Faith Estimate for the cost of our services upon request. This estimate will include the expected costs of services you will receive. Please note that the actual cost of therapy may vary depending on the length and frequency of sessions and the course of treatment.
While the Good Faith Estimate is not a contract and does not obligate you to receive services, it does serve as a guideline for anticipated expenses. If the total cost exceeds the Good Faith Estimate by $400 or more, you may have the right to initiate a patient-provider dispute resolution process.
If you have questions or need further assistance regarding the Good Faith Estimate, please feel free to contact our office. For more information about your rights under this law, visit CMS.gov/NoSurprises.
